The Student acknowledges the existence of certain inherent risks in martial arts training and hereby agrees to assume all risks. The martial arts training includes: hands and feet to block and strike, Judo/JuJitsu skills including, falling skills, throwing skills,arm and wrist locks, submissions holds, jumping kicks, aerial and acrobatic techniques, sparring and self-defense skills, demonstration. He/she further relieves the Mentor Karate Institute, its management, assigned instructors, and his/her fellow classmates from any liability resulting from personal injury and/or loss of personal property.

The Student further acknowledges that the rules and regulations governing the Institute have been adopted and may be changed from time to time and the student agrees to abide by all such rules and regulations adopted, including the time schedule for instruction and the use of the Institute facilities.

Student participation in activities other than that of regular classes, is doing so without liability to the Mentor Karate Institute or any of its instructors. I understand that the use of protective equipment is required by the Mentor Karate Institute and will largely eliminate the possibility of accident or injury. I waive any case resulting from such activities.

Any pictures that may be taken of students during class, demonstrations, tournaments,seminars, or any other Mentor Karate Institute activity are hereby assigned and authorized to the Mentor Karate Institute, without limitation, the right to reproduce, copy, exhibit or publish such pictures. I waive all rights to financial compensation
